Routine In-Home Nursing Visits
We perform routine in-home wellness visits during which we complete a thorough health assessment with monitoring of vitals. We provide ongoing education so our patients feel informed and confident about their diagnosis, medications, and overall health. Our routine wellness visits are important for recognizing potential changes in health status and for evaluating the patient's ability to safely perform daily self-care.
Patient Advocate
As a healthcare advocate, we help to ensure that our patients maintain their maximum health potential.
We communicate any change in condition and/or need for further care to the patient's treating physicians.
We attend physician appointments with clients and provide advocacy by asking those important questions.
Medication Management
We provide education on use, dosage and delivery while encouraging safe administration and compliance.
We create safe solutions by setting up and maintaining pill planners and other medication delivery systems.
Individualized Care Plans & Personal Healthcare Portfolios
We create individualized nursing care plans and personal health portfolios for each client. The care plans help improve patient outcomes, enhanced engagement, and more effectively manage chronic conditions.
24-Hour Nurse Line
Our clients have access to our 24-hour nurse line.
The nurse can assess symptoms, answer health questions, and offer support and guidance on appropriate care during a potential health crisis.
SPARK© Neurocognitive Program for those with Alzheimer's & Dementia
Our neurocognitive program was developed for those with Alzheimer's & Dementia. It also benefits post stroke patients by incorporating neurocognitive exercises and memory care assistance into care plans to help promote independence with activities of daily living.
Our Comprehensive Assessment for New Clients Includes:

Diabetic Management
We assist with diabetic management by tracking blood glucose levels and trends. We provide education on the disease process including signs and symptoms for self-monitoring. We provide education on insulin pens and pumps while encouraging compliance with a diabetic diet and diabetic foot and eye care.
